PDF Format
Word Doc, ACII Text, or other editable format
Most members will not be able to edit a PDF file, but it is faster to download in most cases.  You can fill it out, scan it, and e-mail it.


Forms in Word doc can be filled in on the computer.  Just tab around or click to the fields you want to edit.  You will not be able to click into areas not intended to be filled in by the user.
